To Report a Quality Complaint

When reporting a quality complaint, it will be helpful if you are able to provide the following information:
- Product name
- Product code
- Lot #
- Date of occurrence
- If there was any user impact
- A detailed description of the nature of the complaint

- Select English
- Click “Continue”
- Click “Search Active Licences”
- Select “Company Name”; enter “Alcon”, then Search Database
- There are 2 Alcon Manufacturing sites listed. The majority of products are located under the Alcon Laboratories, Inc., Fort Worth, Texas, US site, Company ID # 100354
